When do I need to submit my student health form?
Student health forms should be completed by a licensed health care provider and must be submitted to the University Student Health Services Office by mail at P.O. Box 842022, Richmond, VA 23284, or in-person at 1300 W. Broad Street., Suite 2200.
This form should be submitted as soon as possible in order to avoid having a hold placed on the student’s registration. The VCU Certificate of Immunization can be found online at www.students.vcu.edu/health/immunizations/.
